Disease Management
Approximately 15% of employee populations have common chronic conditions with another 5% suffering from a catastrophic illness. A Disease Management Program insures that high-risk, high cost employees are getting the necessary education, support, and medical supplies needed to adhere to their individual treatment plans based on best practices. Positive outcomes equal lower claim costs.
We encourage our Disease Management Nurses to become certified as Chronic Care Professionals (CCP). This certification training focuses on evidence-based interventions for improved provider-patient partnering, chronic disease self-management, treatment adherence, and behavior change facilitation – including motivational interviewing.
Our approach is to provide coordinated health management by Registered Nurses to include: early targeted intervention, realistic patient directed goal setting, education, promotion of self management, and compliance with the treatment plan. Individuals with chronic health conditions are identified, stratified and monitored using our predictive modeling tools.
