Fully Integrated Programs
Our Fully Integrated Programs create a seamless continuum of care. Through communication and coordination between these programs, patients needs are identified and stratified to ensure the appropriate education and treatment. Based on the diagnosis and needs, a specially trained nurse will guide the participant through the health care process.
